Find the equipment that was missing
A member needed a lighter resistance band during shoulder exercises. Ask which station needed another option so the instructor can prepare it for the next circuit class.
Fix the details around a first visit
The class went well, but a new member could not find the entrance or bag storage. Ask about arriving and settling in, then add those directions to your welcome message.
Conditional questions, content and dependent details
Can questions depend on an earlier answer?
Yes. Add a rule to an item in the form to show it or require an answer when another question matches. Save the rule and then the form. For example, show injury details only when the customer answers Yes to an injury question.
Can I include instructions without asking for an answer?
Use an Info box for a callout with an optional emoji, or Paragraph for plain text. Both support separate English and Chinese content and can be shown conditionally.
The item library also includes Add Dependent, which collects Name, Profile picture, Birthday and Notes and creates a dependent under the customer account when submitted. Like a question, it can be shown or required conditionally.

Can I collect comments as well as ratings?
Yes. Include a text question alongside ratings. Ask what helped or what could change so an instructor can understand the reason behind a score.
How is feedback different from a survey?
Class feedback asks about an attended session. Use Surveys to ask a selected group about a future decision, such as whether they want a 7pm class.
